This guide explains the process in practical language so you can compare a fertility clinic, specialist and quotation on more than a headline package price.
In a natural cycle, one follicle commonly becomes dominant. During IVF ovarian stimulation, gonadotropin medication encourages a group of follicles to continue developing so several eggs may be available at retrieval. More follicles do not automatically mean more usable embryos. Follicles can be empty, eggs can be immature, mature eggs may not fertilise, and fertilised eggs may not reach blastocyst.
That is why a sensible stimulation plan is a chain rather than a prescription. Your fertility specialist considers age, AMH, antral follicle count, diagnosis, body characteristics and previous response; chooses a starting strategy; then interprets what happens after treatment begins.
FSH-containing medicines, sometimes with LH activity, are used to stimulate follicle development. The brand matters less to a patient than understanding why the dose was chosen, how it is administered and how response will be checked.
Antagonist or agonist strategies are used to control the hormonal sequence and reduce the risk of premature ovulation. The timing depends on the protocol rather than a generic IVF calendar.
The trigger injection supports final oocyte maturation before retrieval. Timing is critical. You should receive the exact drug, dose, date and clock time in writing, plus instructions for what to do if anything goes wrong.
Patients understandably focus on follicle count, but the clinically meaningful chain continues after retrieval: eggs retrieved, mature eggs, normally fertilised eggs, blastocysts and—where relevant—embryos suitable for testing or transfer.

Agonist protocols use a different sequence of hormonal control and may still be appropriate in selected cases. Mild or lower-dose approaches may be discussed when the aim is to reduce medication exposure or when a high dose is unlikely to recruit meaningfully more follicles.
No protocol is automatically “best” because it is newer, shorter or more aggressive. Ask what the clinic expects your ovaries to do, what evidence from your history supports that expectation and what it would do differently after an unexpectedly low or high response.
The team may continue stimulation, review the dose, reassess the likely yield or discuss whether continuing remains worthwhile. Ask how the clinic defines poor response and what its cancellation policy means financially.
More medication cannot create an unlimited number of recruitable follicles. The decision becomes whether the expected eggs still justify retrieval and how this cycle fits a multi-cycle strategy.
Safety becomes central. The clinic may alter medication, trigger choice or the post-retrieval plan, including considering a freeze-all approach in appropriate cases.
During monitoring, the specialist looks at how many follicles are developing, their sizes and whether the cohort is reasonably synchronised. Depending on the clinic and case, blood tests such as estradiol may add information. The point is to decide whether the current dose remains appropriate and when the follicles are ready for the trigger decision.
For an international patient, monitoring also determines logistics. If you start medication at home, establish who performs scans, what measurements are required, where results are sent, which time zone the Thai team uses for instructions and how quickly the specialist will respond.
The trigger is given at a precise interval before egg retrieval. Clinics may use hCG, a GnRH agonist trigger or another strategy depending on protocol and risk. This is not the dose to take “around bedtime” unless that is the exact written instruction.
Before leaving the clinic, confirm the medication, amount, injection method, exact local time, what time zone applies if you are travelling, and the emergency contact route. If the trigger is late, missed, spilled or administered incorrectly, contact the fertility team immediately.
Medication costs vary because dose, brand mix and number of stimulation days vary. Avoid treating one headline package as your final IVF budget unless the inclusions are explicit.

IVF travel works better when you distinguish fixed dates from provisional dates. The start of medication may be predictable, but follicle response can move the trigger and retrieval. A patient who books a non-changeable flight immediately after an estimated retrieval date can create unnecessary pressure.
If you begin stimulation outside Thailand, ask whether your Thai specialist accepts external monitoring, what scan format is required and whether blood tests are needed. Clarify who gives dose instructions and how rapidly results must arrive. Never make an independent dose change because a scan “looks good” to you.
Retrieval turns follicle response into an actual egg count. The laboratory then tells you how many eggs are mature, how many fertilise normally and how many continue developing. If ICSI, blastocyst culture, freezing or PGT is planned, those steps can affect both the treatment calendar and the final cost.
Patients often receive a bag of medicines and a calendar, but understanding the job of each stage makes the plan easier to follow and easier to compare between clinics. The exact brands and combinations vary. What matters is that the specialist can explain why each medicine is present and how it connects to the next decision.
FSH activity drives follicular development. Some protocols also provide LH activity. The goal is a useful cohort, not a competition for the highest follicle count. If a clinic quotes an expected egg number before seeing your response, treat it as an estimate rather than a promise.

For patients at higher risk of OHSS, the medication and trigger strategy can become part of a wider safety plan. That may also influence whether a fresh transfer is sensible or whether embryos are frozen for a later transfer. Ask the specialist to explain this before stimulation rather than only after a strong response appears.
Confirm the start criteria, medication supply, injection training and contact route. International patients should also settle remote-monitoring arrangements and medication storage before leaving the clinic.
Take medication at the instructed time and record doses. Do not adjust because you feel bloated, feel nothing, or have compared your dose with someone online. Symptoms are not a reliable measure of follicle response.
The first useful scan begins to show whether the starting strategy fits your ovaries. A clinic may keep the dose unchanged even if another patient with similar AMH receives a different dose because the pattern and history differ.
As leading follicles grow, monitoring becomes more about maturity, synchrony and safety. This is when travel dates should remain flexible. Ask when the clinic expects to know the likely trigger night rather than demanding certainty too early.
Set alarms, confirm the medicine and keep the clinic's emergency number available. If you are uncertain about the injection, ask before the scheduled time. A time-critical instruction deserves more than a screenshot buried in a messaging thread.
After retrieval, the conversation changes from follicles to eggs and then embryos. Request the retrieval result and understand when the embryology team will update you on maturity, fertilisation and further development.
A previous cycle shows how your ovaries responded to real medication. Bring the protocol name, daily doses, scan measurements, hormone results if available, trigger type, eggs retrieved, mature eggs and embryology outcome. A specialist can then ask whether the next cycle should repeat, intensify, reduce or fundamentally change the approach.
Do not reduce the previous cycle to “it failed.” A cycle with few eggs but good maturity tells a different story from a cycle with many follicles but poor maturity, or a cycle with good egg numbers followed by low fertilisation. The point of records is to locate where the attrition occurred.
Stimulation can cause bloating, pelvic heaviness, headaches, mood changes, bruising or tenderness around injection sites. Your clinic should explain what it considers expected, what can be managed routinely and what needs prompt assessment. Ask how the clinic selects the starting dose, what monitoring would trigger a change, which trigger strategies it uses in higher-risk patients and when it would recommend freezing embryos rather than proceeding directly to fresh transfer.
Stimulation can affect egg numbers, but age-related egg quality remains a separate issue. Ask the specialist to connect the medication strategy with the likely number of mature eggs and the probability of reaching usable embryos. If PGT-A is being considered, discuss whether the expected embryo numbers make testing useful in your individual case rather than assuming it is automatically required.
Do not accept a protocol change without an explanation of what problem it is intended to solve. If the previous cycle produced few eggs, poor maturity, low fertilisation or poor blastocyst development, those are different problems. Some relate to stimulation; others may point toward sperm, laboratory or embryo-development factors. The next plan should identify the suspected bottleneck before changing medication.
Before the first dose, ask a nurse or clinician to demonstrate preparation and administration using the exact device you will use. Confirm whether the medication is a pre-filled pen, cartridge, powder requiring mixing or another format. Photographing the clinic's written instructions can help, but keep the official dose schedule as the primary reference.
Choose a consistent daily routine around the clinic's timing instructions. Keep medication, needles, alcohol swabs and a sharps container together. Check the label before every injection and record the dose after taking it rather than relying on memory. If you are using several medicines, a simple tick-box schedule can prevent one drug being confused with another.
