Age-related macular degeneration can change the part of vision people rely on for reading, recognising faces, driving and seeing fine detail. The important question is not simply whether you “have macular degeneration”, but which form and stage you have, whether there is active leakage or geographic atrophy, and what treatment or monitoring is appropriate now.

AMD is usually divided into non-neovascular or “dry” AMD and neovascular or “wet” AMD. Dry AMD is more common and may progress slowly through early and intermediate stages. A late dry form called geographic atrophy involves loss of retinal tissue. Wet AMD develops when abnormal blood vessels grow beneath or within the macular region and leak fluid or blood. Wet AMD can damage central vision quickly and usually needs prompt retinal-specialist assessment.
These labels matter because the treatments are different. Anti-VEGF injections are a cornerstone of wet AMD treatment. Selected patients with intermediate AMD may be advised to use an AREDS2 nutritional formulation. Newer intravitreal treatments can slow progression of geographic atrophy in appropriate patients in jurisdictions where they are available, but they do not restore retinal cells that have already been lost.
AMD may initially be found during a routine dilated examination before the patient notices a major problem. Symptoms can include reduced reading clarity, difficulty seeing in dim light, reduced contrast, colours appearing less vivid, a blurred or missing patch in central vision, or straight lines appearing bent or wavy. This distortion is called metamorphopsia.
A home Amsler grid can help some patients notice a change between scheduled appointments, but it does not replace retinal examination or OCT. A sudden new distortion, central dark patch or rapid drop in vision can indicate conversion to wet AMD or another retinal problem and should trigger prompt ophthalmic assessment rather than waiting for a future trip.
Medical tourism should never create delay in an urgent retinal condition. If vision changes suddenly, obtain local eye care first. Thailand can be considered once the immediate clinical position and safe travel timing are understood.
Drusen alone, intermediate dry AMD, geographic atrophy and active wet AMD are not interchangeable diagnoses. The stage determines whether the plan is observation, nutritional advice, injection therapy or another specialist pathway.
OCT can show fluid, retinal architecture and structural change. For wet AMD, treatment decisions are often guided by the relationship between symptoms, visual acuity, examination and serial OCT findings.
Previous scans, injection dates, drug names and visual response matter. A single scan in Thailand is more useful when the retinal specialist can compare it with the patient's earlier course.
A comprehensive retinal assessment may include visual acuity, dilated examination, intraocular pressure and high-resolution optical coherence tomography (OCT). OCT is central to modern macular care because it shows cross-sectional retinal anatomy and can reveal intraretinal or subretinal fluid, pigment epithelial changes and atrophy.
Colour retinal photography can document the fundus over time. Fundus autofluorescence may help characterise areas of retinal pigment epithelium stress or atrophy. Fluorescein angiography may be used when the specialist needs to assess leakage or abnormal vessels, while OCT angiography can provide additional non-invasive vascular information in selected cases.
For an international patient, send previous OCT scans if possible—not merely a written statement saying “AMD”. A sequence of scans can reveal whether fluid is new, stable or recurrent and whether an existing injection interval has been controlling disease.

For early dry AMD, the focus may be observation, risk-factor modification and scheduled retinal review. Smoking is an important modifiable risk factor. A balanced diet, management of general cardiovascular health and regular eye examinations form part of sensible long-term care, but they should not be presented as a cure for established macular degeneration.
The AREDS2 formulation can reduce the risk of progression in selected people with intermediate AMD or certain advanced disease patterns. It is not a universal vitamin for everyone with drusen, and it does not restore vision already lost. The ophthalmologist should decide whether the patient's retinal findings fit the evidence for supplementation and consider other health issues or supplements already being taken.
For former or current smokers, beta-carotene-containing older formulations are avoided because of lung-cancer risk; modern AREDS2 uses lutein and zeaxanthin instead. Patients should bring a complete medication and supplement list to the consultation.
Wet AMD is driven by abnormal blood vessels that leak beneath or within the retina. The standard modern treatment for many patients is an intravitreal anti-VEGF injection. Medicines in this class suppress vascular endothelial growth factor, reducing leakage and abnormal vessel activity. The aim is usually to control disease, preserve vision and, in some patients, improve vision.
One injection should not be sold as a complete cure. Treatment commonly begins with closer review and repeat injections, followed by an interval strategy determined by the retina specialist and the eye's response. Different anti-VEGF agents and dosing strategies exist. The right comparison is therefore not simply “which injection is cheapest?” but which medicine, interval, monitoring plan and practitioner are appropriate for the eye.
Delay matters. Active wet AMD can scar the macula and cause irreversible loss. A patient with new central distortion or sudden deterioration should seek prompt assessment rather than waiting weeks for cheaper travel dates.

A very fine needle delivers medicine into the vitreous cavity. The injection itself is brief. Patients commonly describe pressure or momentary discomfort rather than a prolonged procedure.
Mild surface irritation, watering or a small patch of blood on the white of the eye can occur. The provider should give clear instructions about expected symptoms and urgent warning signs.

Availability and regulatory status can differ by country and provider. The objective is to preserve useful vision and control active disease where possible. A good proposal also explains the limits: established retinal damage may not reverse, injections carry small but important procedural risks, and long-term monitoring remains part of the treatment.
Treatment cannot guarantee recovery of retinal cells already destroyed by atrophy or scarring.
Increasing pain, redness or significant visual deterioration after an injection needs urgent review.

Ask who will personally assess the retina, whether that same specialist determines the injection strategy, which anti-VEGF medicines are routinely available, how treatment response is documented and how rapidly the clinic can see a patient with a post-injection concern.
If you already receive injections at home, provide the drug name, dose if known, dates and previous interval. Switching provider does not mean the clinical history should restart from zero.
Some patients retain useful reading and facial-recognition vision for years; others develop central scotomas despite appropriate treatment because of established atrophy or scarring. When central vision is permanently reduced, low-vision rehabilitation can help patients use remaining vision more effectively.
Magnification, electronic readers, improved lighting, contrast enhancement, accessibility settings and occupational strategies can make a meaningful difference. Peripheral vision is usually preserved in AMD, so rehabilitation focuses on adapting tasks and using the remaining retinal function rather than promising that a device “cures” the macula.

A patient travelling for cataract surgery may have a defined operation and recovery pathway. AMD can be different. Wet AMD may require treatment over months or years, and geographic-atrophy therapy may also involve repeated injections. The commercial and travel plan therefore has to respect the disease's continuing nature.
