Revision hip replacement is not simply a second version of the first operation. It is reconstruction surgery for a hip replacement that has become painful, loose, worn, unstable, infected, fractured or mechanically compromised.
The central question is not only whether the implant needs changing, but why the first hip has failed, how much healthy bone remains, whether infection is present, and what reconstruction will give the safest durable result. This guide takes you through those decisions from diagnosis to recovery and travel planning in Thailand.
A well-functioning hip replacement can provide many years of pain relief, but an artificial joint can fail for different reasons. A revision surgeon needs to distinguish between implant loosening, wear, recurrent dislocation, infection, fracture around the implant, component malposition, corrosion or reaction to wear debris, and pain that is actually coming from somewhere outside the replaced hip.
A painful replacement does not automatically mean another operation. Back problems, tendon irritation, muscle weakness, bursitis and other conditions can mimic implant pain. Revision surgery should follow a diagnosis, not simply an abnormal-looking scan.
The work-up often combines the history of the original replacement, serial X-rays, examination and blood tests. CT or other imaging may be used to understand component position or bone loss. If infection is possible, the team may recommend joint aspiration and microbiology testing before deciding the operation.
If you are travelling to Thailand for a revision opinion, collect as much information as possible before you fly. The original operative report, implant stickers or component details, old and recent X-rays, discharge summaries, infection history, previous cultures and a timeline of symptoms can all change the plan.
Serial imaging is particularly useful because a surgeon can compare the position of the components and see whether radiolucent lines, migration, wear or bone loss are progressing. Knowing the manufacturer and size of an existing component can also help the hospital anticipate extraction tools and compatible parts.

If one component is well fixed and correctly positioned, the surgeon may sometimes retain it and revise only the problematic side or exchange modular parts. The decision depends on compatibility, stability, wear and the reason for failure.
When both the cup and femoral component are loose, damaged, poorly positioned or implicated in the problem, a more extensive reconstruction may be required. Removing well-fixed implants can itself create bone loss, so the plan is highly individual.
When deep infection is confirmed or strongly suspected, the strategy may involve removal of implants, thorough debridement, targeted antibiotics and either immediate reimplantation in selected cases or a staged approach with later reconstruction.
Two revision quotations can describe “revision hip replacement” while covering very different surgery. A useful comparison identifies which components are expected to be removed, the likely implant system, whether augments or grafts may be required, what happens if infection is discovered, expected inpatient care, rehabilitation and the contingency costs if the operation becomes more complex.

Primary hip replacement starts with relatively predictable anatomy. Revision surgery may begin with missing or weakened bone. Removing a loose or well-fixed component can add further defects. Surgeons therefore need implants and reconstruction techniques that can obtain stable fixation beyond damaged areas.
Depending on the defect, this may involve longer femoral stems, porous metal shells, augments, screws, structural or morselised bone graft, specialised liners or other revision systems. In severe cases, the reconstruction can be much more extensive than a standard total hip replacement.
This is one reason to ask whether the hospital routinely stocks the revision systems and backup sizes that the surgeon may need. The best plan anticipates what happens if the actual bone loss is worse than the pre-operative images suggested.
Prosthetic joint infection can sometimes present dramatically with fever, drainage and an acutely painful hip, but it may also be subtle. A low-grade infection can resemble aseptic loosening. Missing it before revision can undermine the reconstruction.
Blood inflammatory markers may form part of the work-up, but no single test answers every case. When suspicion remains, aspiration of the joint and analysis of fluid can help. The surgeon may also plan multiple tissue samples during revision.
Previous wound problems, unexplained pain, prior infections, recent bacteraemia and earlier revision history all matter. Tell the treating team about antibiotics already taken because they can affect culture results.
Ask whether the proposed pathway is debridement with implant retention, a one-stage exchange or a two-stage exchange, and why that approach fits your organism, soft tissues, implant stability and general health.
For a staged reconstruction, understand the spacer, antibiotic plan, mobility restrictions, repeat testing and the criteria used before reimplantation. International patients also need to know whether remaining in Thailand between stages is practical.
Recurrent dislocation can result from component orientation, soft-tissue weakness, impingement, spine-pelvis mechanics or a combination. Simply changing a liner without understanding the cause may not solve the problem.
Component position: imaging can show whether the cup or stem orientation contributes to instability.
Soft-tissue tension: muscle and abductor function influence stability and may affect implant choice.
Spine and pelvis: stiffness or previous spinal fusion can alter functional cup position between standing and sitting.
Implant strategy: larger heads, dual-mobility constructs or constrained options may be considered in selected situations, but each has trade-offs.
A fracture around the femoral stem may occur after a fall or around weakened bone. Treatment depends on where the fracture sits, whether the stem is still securely fixed and the quality of the remaining bone. Some fractures can be stabilised around a stable implant; others require both fracture fixation and revision to a longer stem.
For an overseas patient, this is especially important because mobility, weight-bearing and the time needed before flying may be substantially different from an uncomplicated primary replacement.
Treatment in Thailand often means several appointments, with free days or recovery time in between. Plan where you stay, how you spend those days and when you fly — then send it to JLIT alongside your treatment enquiry.
A useful revision opinion should make the uncertainty smaller. It should explain the suspected failure mechanism, what evidence is still missing, what can be preserved, what may need reconstruction and how the surgeon will respond if the operation is more complex than predicted.
Ask the surgeon to show you the problem on the imaging. Is the component definitely loose? Is there progressive bone loss? Is the pain pattern consistent with the implant? Has infection been reasonably excluded? Are the abductors intact? Could the lumbar spine or another source explain some symptoms?
If the answer is uncertain, ask what test would change the decision. Revision surgery is substantial; “the X-ray does not look perfect” is not the same as a clear indication for reconstruction.
Which components are expected to stay? Which are expected to come out? Is an extended surgical exposure or osteotomy anticipated? What bone graft, augments or specialist revision components may be needed? What is the backup plan if the implant proves harder to remove or the bone defect is larger?
Ask what the operation is intended to improve: pain, stability, leg function, infection control, fracture healing or prevention of further bone loss. Revision can be very successful, but expectations should reflect the starting problem and the complexity of reconstruction.
International treatment adds practical decisions that local patients do not face. Ask how long you should remain near the hospital, when the wound will be reviewed, whether stitches or staples need removal, what mobility aid you will use, when you can tolerate a long flight and who handles concerns once you are home.
Optimisation matters because revision surgery can involve longer operating time, more blood loss and a more demanding recovery than primary hip replacement. Your hospital may review anaemia, diabetes control, nutrition, kidney and heart health, medications, smoking, dental or skin infection risks and other factors before surgery.
Medication instructions need to be specific. Anticoagulants, antiplatelet drugs, diabetes medicines and supplements may require individual plans. Do not stop prescribed medication simply because you are travelling for surgery; follow the treating team's instructions.
At the accommodation level, think beyond the hotel photograph. You may need a walk-in shower, lift access, space for a walker, a firm chair, easy transport to review appointments and a companion for the early period.
Revision surgery is performed under anaesthesia with antibiotics and blood-clot prevention planned around your individual risks. The surgical approach depends on the existing scar, components and reconstruction. The team first gains safe access to the joint, assesses tissues and implant fixation, and takes samples if infection is a concern.
Components selected for removal are extracted while preserving as much bone as possible. Defects are then reconstructed and the new components fixed. In complex cases, the final implant choice may only be confirmed once the surgeon can see the true condition of the bone.
After surgery, pain control, blood tests, wound monitoring, mobilisation and physiotherapy begin. Some patients can bear weight relatively early; others need partial or protected weight-bearing because of bone grafting, fracture fixation or the type of reconstruction. Your own restriction overrides generic recovery timelines.
Control pain, monitor the wound, prevent clots, practise transfers and walking with the prescribed aid. Follow any hip precautions and weight-bearing restrictions.
Walking distance and basic function gradually improve. The pace depends heavily on reconstruction complexity, muscle condition and whether the surgeon has restricted loading.
Progressive strengthening, gait work and balance become more important as healing permits. Complex revisions may deliberately advance more slowly.
Improvement can continue for many months. The goal is dependable everyday function rather than chasing an artificial deadline copied from primary hip replacement.
A long-haul flight soon after major lower-limb surgery adds immobility, swelling and practical difficulty at a time when the risk of venous thromboembolism is already being managed. Revision surgery can also need closer wound and radiographic follow-up than a straightforward primary procedure.
Plan the stay around the surgeon's expected review schedule and your individual mobility rather than a fixed holiday duration. Consider airport assistance, an aisle seat if appropriate, medication timing, compression or anticoagulation instructions, luggage handling and transport at both ends.
If you develop increasing wound drainage, fever, worsening redness, sudden severe pain, a new deformity, calf swelling, chest pain or shortness of breath, seek urgent medical assessment rather than waiting for a scheduled flight.
Revision hip replacement is often longer and technically more demanding than primary surgery. Scar tissue changes the surgical planes, components may be difficult to remove, bone may be thin or missing, and the soft tissues that stabilise the joint may already have been disturbed. This is why a surgeon's experience with revision work matters independently of their experience with routine primary hip replacement.
Potential complications include infection, blood clots, bleeding or transfusion, fracture during component removal or insertion, dislocation, nerve or blood-vessel injury, leg-length difference, persistent pain, wound problems and failure of the new fixation. There is also the possibility that an implant expected to be retained proves loose or damaged once the joint is exposed, expanding the operation.
These risks are not a reason to avoid a necessary revision. They are a reason to understand the indication, optimise health before surgery and choose a team with the equipment and backup systems for complex reconstruction. Ask how the hospital manages unexpected blood loss, difficult implant extraction, major bone defects and intensive-care needs if they arise.
Lower-limb joint surgery increases the risk of deep-vein thrombosis and pulmonary embolism. Your prevention plan may combine early mobilisation, mechanical measures and anticoagulant medication. The exact drug and duration depend on your medical history, bleeding risk and surgeon's protocol. For a traveller, the plan should also address the prolonged sitting associated with the journey home.
Revision surgery can relieve pain and restore useful function, but the expected result varies. A patient having a straightforward exchange for a loose component with good bone stock starts from a different position from someone undergoing reconstruction after repeated infection, severe bone loss or multiple previous operations.
Ask the surgeon to describe the expected outcome in practical terms. How far might you reasonably walk? Are stairs likely to become easier? Is the objective to return to recreational activity, to eradicate infection, to stabilise a repeatedly dislocating hip, or to make a fracture heal? A clear functional target makes the recovery easier to judge.
It is also useful to discuss what might remain imperfect. Muscle weakness can persist, particularly when the abductors have been damaged. A leg may not feel identical to the other side. Some patients continue to notice stiffness or discomfort even when the reconstruction is mechanically sound. Complex revision is often about achieving a stable, comfortable and dependable joint rather than recreating a completely normal hip.
Before leaving, obtain the operative report, implant details, discharge summary, medication list, latest X-rays, pathology or culture results where relevant and the rehabilitation instructions. Ask when new X-rays are due and whether the Thai surgeon expects to review them remotely. Your doctor or orthopaedic team at home should know exactly what was implanted and whether there are specific precautions.
The surgeon usually works with native anatomy, removes the damaged joint surfaces and inserts components into bone that has not previously held an implant. Component choice is important, but the range of likely reconstruction is comparatively predictable.
Rehabilitation protocols are often standardised because the bone and soft-tissue situation is familiar. Many patients can follow a relatively consistent mobilisation pathway, although individual health still matters.
The surgeon starts with an existing implant, scar tissue and potentially altered anatomy. The component may be loose or extremely well fixed. Bone can be missing around the cup or stem. Infection, fracture or instability may add another layer of complexity.
The operation therefore needs a primary plan and contingency plans. Weight-bearing and recovery may depend on what is found during surgery and how the reconstruction achieves fixation.
Revision hip replacement cannot always be priced as neatly as a routine primary operation because the exact reconstruction may depend on findings at surgery. The quotation should still be transparent about the assumptions used. Ask which implant system and components are included, whether specialist augments or bone graft are allowances or additional charges, how many inpatient nights are included and how complications or extra nights are billed.
Infection pathways can create very different costs from aseptic revision. A two-stage exchange can involve two operations, a temporary spacer, prolonged antibiotics, repeat investigations and a much longer overall treatment period. A periprosthetic fracture may require plates, cables or other fixation in addition to revision components. These are materially different treatment episodes and should not be compared by headline price alone.
Also separate medical costs from the travel layer: accommodation before and after admission, companion costs, airport assistance, accessible transfers, physiotherapy outside the hospital, medication after discharge and a possible extension of stay if the surgeon does not clear you to fly on the original date.
Early independence is built around safe transfers and short walks rather than distance. A walker or crutches may be necessary, and the surgeon may restrict how much weight you put through the operated leg. Do not discard the aid simply because the hip feels comfortable; restrictions may be protecting grafts, fracture fixation or new component fixation.
Stairs are usually taught by the physiotherapy team before discharge if you need them. Sleeping position and hip precautions depend on the surgical approach and stability of the reconstruction. Use the instructions from your own team rather than generic rules from another patient's primary replacement.
Driving requires more than being able to sit in a car. You need adequate control, reaction time, strength, safe entry and exit, and you should no longer be impaired by strong pain medication. Insurance and local legal requirements also matter. For many international patients, driving is not relevant until after return home, but ask the surgeon for a clinical milestone rather than guessing.
Desk-based work may be possible earlier than physically demanding employment, but prolonged sitting can still be uncomfortable and travel to work may be difficult. Jobs involving lifting, climbing, uneven ground or prolonged standing require a more advanced level of recovery.
High-impact activity may be discouraged after a revision, particularly when bone stock is limited or the reconstruction is complex. Lower-impact exercise such as walking, controlled cycling or swimming may eventually be more appropriate once the wound has healed and the clinical team has cleared progression.
If returning to golf, gym training or another specific sport matters to you, say so before surgery. The surgeon and physiotherapist can explain whether that goal is realistic and which movements or loads should be modified. The answer may differ significantly from advice given after an uncomplicated first hip replacement.
Price matters, but the safest comparison starts with whether the proposals describe the same clinical problem and the same level of reconstruction.
One component, both components, modular parts, fracture fixation or an infection pathway should be explicit.
Ask about revision stems, cups, augments, grafts and the financial implications of contingencies.
Clarify surgeon, anaesthesia, theatre, implants, inpatient nights, medicines, physiotherapy, investigations and follow-up.
Understand extra hospital nights, wound care, repeat imaging, physiotherapy and communication after you return home.
The aim depends on the failure mechanism. A loose implant causing mechanical pain may improve substantially, while complex soft-tissue or nerve-related symptoms may not disappear completely. Ask what symptoms the surgeon expects the reconstruction to change.
Many patients mobilise early with assistance, but weight-bearing can be restricted after complex reconstruction, grafting or fracture treatment. The operative plan determines the rule.

There is no universal date suitable for every revision. Your surgeon should consider clot risk, wound healing, mobility, reconstruction and the length of the journey before clearing travel.
Longevity varies with the reason for revision, bone quality, implant fixation, infection history, activity and reconstruction complexity. Ask for an expectation based on your case rather than a generic percentage.
It is particularly reasonable when the cause of pain is uncertain, infection remains possible, major bone reconstruction is proposed or different surgeons recommend materially different operations.
