Shoulder arthroscopy is not one operation. It is a keyhole surgical approach that can be used to inspect and treat several different causes of pain, weakness, catching and instability — and the value of the operation depends on getting the diagnosis and the treatment target right before you travel.
This guide takes the decision from symptoms and imaging through procedure choice, recovery, rehabilitation and planning treatment in Thailand, with the practical detail an international patient needs before committing.
A painful shoulder can come from the rotator cuff, labrum, biceps tendon, cartilage, capsule, bursa, acromioclavicular joint, instability or referred pain from the neck. Several abnormalities may appear on a scan at the same time, particularly with age, so the presence of an MRI finding does not automatically prove that it is the source of symptoms.
A useful surgical opinion connects the history, examination and imaging. The clinician should understand where the pain is felt, whether it is linked to overhead activity, whether the shoulder catches or feels unstable, whether there has been trauma, what strength has been lost, how sleep is affected and what rehabilitation has already been attempted.
For an international patient this matters twice: first because the operation needs to be appropriate, and second because the likely procedure determines how long you may need a sling, how quickly physiotherapy progresses and how sensible a particular flight-home date will be.
One person may have a relatively limited procedure to address inflamed tissue or inspect a joint. Another may need a labral repair for instability, biceps treatment, rotator cuff repair or several procedures during the same operation. Those differences change pain, sling use, movement restrictions, physiotherapy and return-to-work timing.
That is why a quotation for “shoulder arthroscopy” is not enough on its own. Ask what the surgeon currently expects to do and what additional procedures might become necessary if the arthroscopic findings differ from the scan.
The travel plan should be built around the more restrictive realistic scenario rather than the quickest possible recovery.

The assessment normally starts with history and examination. Plain X-rays can be useful for bone, joint-space and alignment questions, while MRI or other imaging may be used to evaluate soft tissues. The appropriate test depends on the suspected problem and prior investigations.
If you already have scans, obtain the actual image files as well as the written reports. A specialist reviewing only a report loses information. Include previous operations, injections, physiotherapy, medication, relevant medical conditions and a concise description of what the shoulder prevents you from doing.
Ask what non-operative treatment remains reasonable. Many shoulder problems are initially managed with activity modification, targeted physiotherapy, pain management and sometimes injections. Surgery becomes a different discussion when symptoms persist despite appropriate treatment, when there is structural damage for which repair is indicated, or when instability or functional loss changes the balance.
Symptoms are improving, the diagnosis does not show a clear surgical target, strength and function remain acceptable, or a structured rehabilitation programme has not yet been completed.

Shoulder scans commonly contain several findings. A second specialist view can test whether the proposed procedure addresses the symptom-generating problem and whether every proposed component is necessary.
Chronological age is only one factor. Tissue quality, arthritis, activity goals, medical health, the type of lesion and realistic rehabilitation commitment can matter more to the treatment choice.
Shoulder arthroscopy is performed through small portals through which the surgeon introduces a camera and specialised instruments. Anaesthesia and positioning depend on the planned procedure and the clinical team. The joint and surrounding spaces can be inspected and the planned treatment carried out.
The visible incisions may be small, but that does not mean the internal procedure is minor. A repaired tendon or labrum still needs biological healing. Patients should judge the recovery by the tissue treated, not by the size of the dressings.
Before surgery, ask which procedures are definitely planned, which are possible depending on findings, and whether your consent and quotation deal clearly with those contingencies.

Before travelling, ask for the expected protocol in writing: sling duration, passive and active movement restrictions, when physiotherapy starts, what exercises are allowed, when strengthening begins and what milestones are used for work and sport.
If you will continue physiotherapy in your home country, the Thai team should provide an operative summary and rehabilitation instructions that another therapist can follow.
Potential complications include infection, bleeding, stiffness, nerve or blood-vessel injury, blood clots, anaesthetic complications, persistent pain and failure to achieve the intended improvement. Procedure-specific risks may include recurrent instability, failed tendon or labral healing, anchor or implant problems, or the later progression of arthritis.
The most important limitation is that surgery cannot reliably reverse every source of shoulder pain. Established arthritis, poor tissue quality, complex chronic damage or pain generated outside the shoulder may alter expectations.
Ask the surgeon what outcome is realistic for your particular shoulder: pain reduction, improved stability, better strength, return to a particular sport or simply improved everyday function.

If a sling is required, dressing, showering, preparing food and sleeping can be inconvenient, particularly when the dominant arm is treated. A companion may be valuable during the early phase. Choose accommodation where daily tasks do not require lifting luggage, reaching overhead or negotiating difficult stairs.
Sleep is often one of the most frustrating parts of early shoulder recovery. Patients may be advised to sleep more upright initially or use pillows to support the arm, but follow the treating team’s instructions. Plan clothing that is easy to put on, and arrange luggage so you do not have to lift a suitcase with the operated arm.
Driving should not be assumed simply because pain is controlled. Sling use, reaction ability, medication, movement and the ability to control a vehicle safely all matter. Obtain explicit advice from the treating clinician and consider insurance requirements at home.
Desk work may be possible before manual work, but even computer use can be uncomfortable in a sling. Jobs involving lifting, pushing, pulling, overhead work or safety-critical tasks generally require more recovery and functional clearance.
Gym and sport progression depends on what was done. A swimmer, tennis player, golfer, weightlifter and office worker place very different demands on the shoulder. Ask for milestones linked to strength, range, control and tissue healing rather than a single universal date.
If your main goal is return to a sport, tell the surgeon before the treatment plan is finalised. That goal may influence rehabilitation priorities and the way success is measured.
Imaging is useful, but arthroscopy provides direct visualisation. Occasionally the exact extent of cartilage, labral, biceps or cuff pathology becomes clearer during the operation. The important issue is not that the plan can change; it is whether the reasonable possibilities were discussed beforehand.
Ask which additional procedures might be performed, which would require separate consent, whether implants or anchors could add cost, and how a more extensive repair would change the sling, rehabilitation and intended travel date.
For medical travel, this contingency planning is particularly important because an unexpected repair may turn a short recovery plan into a longer protected phase.
Do not compress the whole decision into “yes or no to surgery”. Work through the clinical problem, the procedure, the rehabilitation and the international-patient logistics separately.

The result depends on protection and progressive rehabilitation long after the small skin incisions have healed.

There is no single safe number of days in Thailand after shoulder arthroscopy because the operation may range from a relatively limited procedure to a substantial repair. The treating surgeon should decide when postoperative review is needed and when travel is medically appropriate.
Long-haul travel adds prolonged sitting, luggage handling, medication timing and access-to-care considerations. If your arm is protected in a sling, arrange assistance with bags and transfers. Keep essential medicines and clinical documents accessible rather than in checked luggage.
Before leaving Thailand, obtain the operative report, discharge summary, medication list, wound instructions, rehabilitation protocol, emergency contact details and a clear follow-up plan.
Your treating team should give you its own warning signs and contact pathway. In general, worsening wound redness or drainage, fever, escalating pain that is not behaving as expected, new neurological symptoms, chest pain, breathlessness or symptoms concerning for a blood clot warrant prompt medical assessment.
Do not rely on an internet article to decide whether a postoperative symptom is safe. When you travel for surgery, make sure you know who will review you in Thailand and who will take over care after you return home.
That continuity plan is part of provider comparison, not an administrative detail to leave until discharge.

Plain X-rays are useful for the bony shape of the shoulder, arthritis, joint-space loss, previous injury and some features that can alter whether arthroscopy is a sensible option. MRI can show the rotator cuff, labrum, biceps region, cartilage and other soft tissues, although the importance of a reported abnormality still has to be interpreted alongside symptoms and examination.
For some questions the specialist may request a different study or additional views. The important point for a patient is not to collect every possible scan, but to have the right imaging for the suspected pathology. If your existing scan is old and symptoms have changed, ask whether repeat imaging would materially change the plan before paying for it.
During examination the clinician may compare active and passive movement, strength, stability, tenderness and movements that reproduce symptoms. Neck symptoms, nerve symptoms or pain patterns inconsistent with a shoulder lesion may prompt a broader assessment. A good consultation should leave you understanding why the proposed structural diagnosis fits your particular symptoms.
Advanced joint arthritis, major bone loss, some irreparable tendon problems, certain fractures and other complex conditions may be better treated with a different operation or with non-operative management. Equally, pain without a convincing surgical target may not improve simply because a camera can be placed inside the joint.
Ask the specialist what finding would make them advise against arthroscopy. That question is useful because it reveals the boundaries of the proposed treatment. It also helps distinguish a procedure selected for a defined clinical reason from surgery being offered simply because conservative care has become frustrating.
If there is significant arthritis, discuss whether arthroscopy is expected to provide durable benefit or whether it risks delaying a more appropriate treatment. If there is a large chronic cuff tear, ask about tissue quality and repairability. If instability is the problem, ask whether bone loss changes the operation required. These distinctions matter to both outcome and cost.
Your hospital will give procedure-specific instructions. These may include medication review, blood tests or other pre-operative assessment, fasting instructions and planning around medical conditions. Tell the clinical team about prescription medicines, over-the-counter products and supplements rather than stopping anything independently.
Practical preparation matters too. Arrange clothing that opens at the front, organise your recovery space, move frequently used objects below shoulder height and decide who will handle luggage and transport. If the dominant arm is being treated, rehearse basic tasks with the other hand.
Confirm exactly which medicines should be continued, changed or withheld and when they should be restarted.
Discuss modifiable risks with the treating team. Healing and anaesthetic planning are influenced by overall health, not only the shoulder.
Plan for dressing, meals, transport, shopping and other tasks that may be awkward while the arm is protected.
Know where the first sessions will happen and who takes over when you return home.
The cost of shoulder arthroscopy can vary because the phrase covers different procedures. A diagnostic or limited debridement case is not financially equivalent to a repair requiring anchors, longer theatre time and a more structured follow-up programme. A useful quotation therefore describes the assumed procedure rather than presenting a number without clinical scope.
Ask whether the quotation includes surgeon and assistant fees, anaesthesia, operating theatre, hospital or day-case charges, implants and disposable equipment, medicines, pre-operative tests, postoperative imaging if required, follow-up consultations and physiotherapy. Also ask what happens financially if the surgeon finds pathology requiring an additional procedure that was discussed as a possibility.
For an international patient the medical budget is only one column. Recovery accommodation, companion costs, local transfers, physiotherapy, flight flexibility and a longer stay if the surgeon advises against travelling on the original date should all be considered. A cheaper surgical quote can become a poor comparison if it excludes items another provider has bundled transparently.
Return to sport is not a single date because different procedures and sports load the shoulder differently. The path back should reflect tissue healing, movement, strength, control, symptoms and the demands of the activity.
Swimming, tennis, badminton, volleyball and throwing sports require progressive overhead control and endurance. A pain-free daily shoulder is not automatically ready for repeated high-speed overhead load.
Gym progression should respect the repaired structure and rehabilitation phase. Heavy pressing, pulling or unsupported loading should not be restarted simply because the incisions have healed.
Sports where the shoulder may be struck, forced into an unstable position or used to break a fall require additional confidence, strength and clinical clearance.
